NORTH CAROLINA - Buxton, NC: A man died on a beach when an 8-foot-deep hole he had dug into the sand caved in as he sat inside it. Beach-goers said Daniel Jones, 21, dug the hole for fun, or protection from the wind, and had been sitting in a beach chair at the bottom Thursday afternoon when it collapsed, burying him beneath 5 feet of sand. People on the beach on the outer banks, used their hands and shovels, trying to claw their way to Jones, a resident of Woodbridge, VA, but could not reach him. It took rescue workers using heavy equipment almost an hour to free him while about 200 people looked on. Jones was pronounced dead at a hospital.

Pregnant Germans seek cash bonus

German mums-to-be are avoiding actions that may bring on labour
Many German mothers-to-be are reportedly trying to delay labour so their births coincide with a generous new government scheme.
Parents of babies born on or after 1 January will be entitled to up to 25,200 euros (£16,911, $33,300) to ease the financial burden of parenthood.

But those born even a minute earlier will not be covered by the scheme.

The cash subsidies are part of a government initiative to boost Germany's dwindling birth rate.

German women have an average of 1.37 children, well below the average of 2.1 needed to keep a population stable. One minister recently warned of "the lights going out".

Under the current system of Elterngeld, parents receive a maximum of 7,200 euros (£4,831, $9,472) over two years.

But the parents of children born in 2007 will be granted over two thirds of their former salary for up to a year - up to 25,200 euros.

'Let nature take its course'

Doctors have been warning women not to take any medication to try to delay labour, and few, they stress, would put the life of their baby at risk for the sake of the money.

But what many mums-to-be do in order to bring on labour, pregnant Germans are now anxious to avoid.

These include drinking red wine, eating curries and taking part in physical activity.

Midwives are also advising women to avoid cinnamon and cloves - a staple of German Christmas cooking.

And it was to the government's festive spirit that a Berlin bishop appealed this week when he asked for the start date for the new benefits to be brought forward.

"It would be an anti-bureaucratic act in the spirit of Christmas to move the date from 1 January to 24 December," Wolfgang Huber, a leader of the Protestant church, said in the Berliner Morgenpost newspaper.

My name is Andrews, and I'd like to share my story with you. It's a story of despair, hope, loss, sadness, longing and finally joy—not one, but two miracles!

Ten years ago my husband Lindsay and I decided it was “time to start a family”. We had been married for five years, had saved up and bought our first house. We both had stable jobs. I was 34. My husband was 27. We were ready. We made the decision on our fifth wedding anniversary, 9 September 1994.

We thought it would be easy. After all, we'd put so much effort into not getting pregnant over the first five years of our marriage. We'd tried out all of the different contraceptive options, since the contraceptive pill didn't seem to agree with me.

We both had physical check ups. Our GP told us we were both in good health and, despite the fact that I'd left my run a little late, should have no trouble conceiving. I (arrogantly) told my GP I was a very young 34, and we enthusiastically started on our mission.

We were both convinced that it would happen quickly. After all, we were both very goal oriented people. Anything we'd set ourselves as a goal before, we'd always achieved. So we were puzzled when 3 months went by and my period rolled around yet again.

We told ourselves that it would happen when the time was right, and continued our efforts in earnest.

It was only later (years later) that I discovered that many of the things we were doing at this stage were actively preventing us from conceiving and carrying a child.

After 12 months of frustration, the situation was starting to affect our relationship. I was moody and short tempered. I seemed to be frequently on the verge of tears, especially if I was out shopping and saw a pregnant woman, or one of the endless number of strollers and prams with a cute baby or toddler on board.

What was I doing wrong? How come all these other women could do it and I couldn't? What was wrong with me?

I started to feel guilty for leaving it so late. I felt guilty for not looking after my body better—maybe if I'd never drunk alcohol? Maybe I should leave my job?

And I started to blame Lindsay, my husband. What if he was “shooting blanks”? Would it undermine his masculinity if he found out? Would he even go for the test?

Eventually we got all these feelings of resentment and fear out in the open. We had a long heart to heart talk, and decided we needed to get some answers.

I'd been putting off going to the OB-GYN for fear of what I'd be told. I had a good friend who'd been going through IVF (unsuccessfully) and the thought of going through the same thing terrified me.

So Lindsay had his sperm count tested. I had an ultrasound and some blood tests to check for endometriosis or other physical problems, as well as hormonal or other physiological causes.

The results, when they came back, were more frustrating that ever: there was no physical, physiological, or biochemical reason we shouldn't be able to conceive. We, like many couples, had what was termed “non-specific infertility”. In other words the experts didn't have a clue. They suggested we “de-stress” and “have more sex”.

We took a week off and went to the beach. But sex was no longer much fun. It was something we did at the right time because my temperature graph said I was ovulating. Afterwards we'd talk quietly about our fading hopes. We'd try and look on the bright side—just think about all the money we'd save by not having kids. And we'd be able to travel, and go to restaurants whenever we wanted to, and buy a sports car. Who were we kidding? Often after sex we'd just lie together and cry until we fell asleep.

After two years of hope fading to frustration, fading to sadness and emptiness, I knew I had to do something or I would go crazy. I started to research. I read every book, every journal, every article I could get my hands on. I read about western medicine and alternative therapies. I talked to people about new treatments and found out as much as I could about clinical trials that were happening around the world.

I was obsessed. I would overcome this “problem” if it killed me.

I applied everything I learned as much as I could. I was taking 14 different vitamin and mineral supplements every day. My husband was too, as I'd discovered that pre-conception care for the potential father was as important as for the soon-to-be-mother. We were exercising an hour and a half every day. We were eating only organic foods, drinking 2 litres of filtered water every day, eating meat only occasionally (and then only organic), and avoiding anything toxic like alcohol or passive cigarette smoke. We had tests for levels of lead in our systems (we live near a busy road, in a 100 year old house that's full of lead-based paint). I had my amalgam fillings replaced with ceramic to be sure I wasn't affected by mercury. We even took the phase of the moon into account as I'd discovered something called the “biorhythmic lunar cycle”, in which research seemed to show that a woman is at her most fertile during her lunar peak, ie at the same phase of the moon that was present at her birth!!

My long-suffering husband took all this in his stride. In fact I think if I'd told him we'd be more likely to conceive if we had sex underwater, at high tide, on the winter solstice, he'd have done it. We were that desperate.

On 27 May 1999, four years, eight months and 18 days after we'd first made the decision to try for children, we fell pregnant. My hands shook as I took the little urine-soaked strip of plastic with its TWO stripes (TWO STRIPES!!!) to show my husband. We laughed. We cried. We calculated birth dates (28 February 2000—would we have a “leap baby?”). We planned. We giggled like teenagers every time we looked at each other. We'd DONE IT!

We knew it was early days, but we had to share the good news with someone! At seven weeks we told both sets of parents. They were ecstatic.

At nine weeks we turned up for the first ultrasound, excited about the prospect of “seeing” our new baby for the first time. We'd already chosen a name. “Eden” was to be our first-born.

Our OB-GYN was a highly experienced specialist in his late 50s. As part of my research, I'd tracked down the “best in the business”. He looked at the ultrasound, and then looked at us, and said “This is not looking good.” My heart sank. I could feel a knot in my stomach and the tears starting. My husband had my hand in a vice grip. “The heart beat is slow and irregular. Rarely does a fetus in this condition last past 9 weeks.” They were words I just didn't want to believe. I hung onto the next sentence: “Sometimes it sorts itself out, and you may go on to deliver a healthy child, but you shouldn't get your hopes up.” But the next sentence was what gutted me: “Besides, you're 39. The chances of you having a baby at your age are very slim. You should have started in your 20s.” Expert he may have been, but tact and bed-side manner obviously weren't skills he'd honed.

I took two weeks off work, and stayed in bed. It only gave me more time to think and worry. At the end of the ninth week of my pregnancy I started to bleed. I didn't want to believe it. I called my husband, who came straight home. We went up to the doctor's surgery. With a kind of “I told you so” attitude he carried out another ultrasound, then booked me in for a D&C (Dilation and Curettage, where they scrape out the “products of conception” under anesthetic) the next day. I felt violated. Eden was dead. We asked afterwards if we could at least see our baby (our Eden). We were told by the surgeon that “It looks just like a piece of spaghetti. There's nothing to see.” We were numb.

I talked to a mid-wife friend of mine, who gave me a small glimmer of hope. She pointed out that at least now I knew I could fall pregnant. It had taken over four and a half years, but I had overcome a major hurdle. The next barrier was holding the fetus for nine months. She said that with all our pre-conception efforts, we had at least made progress. She said that many women who had a miscarriage were happily pregnant by the time the due date of the miscarried fetus came around. I found it hard to believe, but I hoped.

It was shortly after that, that I discovered a very important piece of research. I believe now that this was the key that finally meant I could fall pregnant and carry a baby to term.

I believe that it was the key reason that we now have two happy, healthy, energetic, challenging, wonderful children. And I'm so grateful that I discovered it.

Because once I discovered this critical piece of information, and applied it, everything fell into place…

On 24 September 1999 our daughter Lauren was conceived. She was born on 4 July 2000. She was a healthy 10 pound 7 ounces, and took to breast-feeding voraciously.

Part of the research I'd done said that babies that were born using the method I'd discovered were often healthier, and matured more quickly than other babies. This was confirmed for us when Lauren started sleeping through the night from 2 weeks of age. She said her first word at 5 months (“Hello”—we got it on video). And at three she was already reading and writing with the ability of a six year old.

When Lauren was 17 months old we decided it was time to try for our second baby. I didn't want to leave it much longer, as by this stage I was 41, and I knew the risks of having a Down's Syndrome baby, or some other congenital defect, was greatly increased as I got older.

On 5 January 2002 our second baby Jacob was conceived.

This time, it took us only one month from when we made a decision to try, to when we actually conceived.

He was born on 24 September 2002. At 9 pound 4 ounces he was a bit smaller than his sister, but also healthy, hungry, and alert. By nine months of age he was communicating with us in deaf-sign language (a friend of ours taught him, and he loved it!). Like his sister, he's continued to develop and learn extremely quickly.

So what was this secret that made all the difference? Why was it that it took us over four and a half years to conceive the first time, only to miscarry?

What did we do so differently the next time, that our first child took only 3 months to conceive, and our second child took only 1 month to conceive—when I was aged 41!

One step nearer to a diabetes cure

It is more than 80 years since insulin was first used in the treatment of early-onset diabetes, but it was never a 'cure' in the sense that it treated the illness once and for all.

Such a cure, long dreamed of, would not only prevent the long-term complications of diabetes, such as kidney failure and blindness, but relieve sufferers of the need to inject themselves once, twice or three times daily.

One possible cure is to transplant the pancreas, which contains the cells that produce the insulin that early-onset diabetics lack. However, the number of diabetics greatly exceeds the possible number of organs for transplant (unless one day animal organs can be used). Therefore another method must be found.

One step towards a possible cure is the injection of the cells that actually produce insulin into the portal vein, rather than transplantation of the whole pancreas. A trial of this treatment was published in the New England Journal of Medicine last month.

Thirty-six diabetics with unstable diabetes underwent the treatment. Because of its complexity, serious side-effects were common, caused both by the procedure itself and the subsequent immunosuppression necessary to allow the insulin-producing cells to be accepted by their new host.

However, some of the results were encouraging and 16 of the patients were able to do without insulin for at least a year afterwards.

The San Diego Postpartum Health Alliance provides a "Warmline" volunteer phone service and therapeutic referrals to struggling new mothers. PHA provides self-assessment quizzes to help determine whether the exhaustion, sleeplessness, sadness and anxiety are normal or symptoms of such conditions as postpartum depression or panic disorders.

According to Mary Obata, Health Alliance president, "A lot of moms feel shame bringing this up with their health care providers. But they can get support and can get better with professional care. They need to feel heard."

Welcome Home Baby, a first-time-parent support program of Palomar-Pomerado Health Systems, hopes to fill gaps in new parents' knowledge in military and civilian families. Annamarie Martinez coordinates the free program, funded by grants from the San Diego First-Five Commission.

"We receive moms' birth information from the birth hospital," which will soon include Balboa Naval Hospital, Martinez said. "Then in the first week, we make contact with the parents and provide a home visit from a registered nurse."

Nurses assess the health of mothers and babies and answer questions about breastfeeding, health and newborn care. But the program does not end there.

"We make phone contact with the mom three or four times throughout the first year of baby's life," Martinez said. "We can also refer moms to social workers, health educators, registered dietitians and lactation consultants as needed."

Help plentiful for struggling new moms

Carlsbad resident Holly Herring was a 12-hour-a-day single career woman when she became pregnant with her now 8-year-old son. A take-charge kind of person, she was sure she would seize her new role with the same energy she had brought to her career.

She wrote, "I find no joy, no pleasure and no love being a mother ... I can't stand to wake up much longer ... I am scared of my feelings."

Jeanne Phillips, who writes as Dear Abby, responded, "You may be suffering from postpartum depression," and advised her to get help and a respite from mothering. In September, Abby followed up with a column about the vast numbers of people in San Diego and nationwide who had written to offer help.

Once again voters have an opportunity to defeat an initiative that threatens the safety of thousands of California's teens. Proposition 85, the so-called Parental Notification Initiative, will appear on the November General Election ballot. This initiative is almost identical to last year's Proposition 73, which was a failed effort by the same sponsors.

Last year the North County Times wisely rejected the parental notification initiative, calling it "a hurdle, plain and simple, placed before pregnant girls by people who want to roll back abortion rights more broadly."

I could not agree more. While backers of 85 are being careful not to discuss their broader agenda in public, their internal newspapers openly reveal Prop. 85 to be a calculated assault on legal abortion and the Roe vs. Wade decision.

While parental notification laws may sound reasonable, in the real world, they are the wrong answer for California and will put thousands of our most vulnerable teens at risk. Parents rightfully want to be involved in their teens' lives, but no law can mandate family communication.

The good news is that a majority of teens already involve their parents in decisions about an unintended pregnancy. As the North County Times said last year, "California's girls have been doing fine without parental notification. Rates of teen pregnancies and abortions have been falling for years. Studies suggest that most girls tell their parents already."

Interestingly in the year since that was published, teen pregnancy and abortion rates have fallen even further, and more than 70 percent of teens now involve their parents in these decisions. According to an August report by the state Department of Health Services, teen pregnancy rates in California have dropped by 46 percent over the last 10 years. This drop occurred without laws like Prop. 85.

But for the remaining teens who don't involve a parent, there is generally a very good reason why they can't. In the real world the teen may fear being kicked out of her home or face violence should she disclose to her family that she's pregnant. Or worse, a family member may have caused the pregnancy. These teens can't go to their parents. Prop. 85 could force these vulnerable teens to delay medical care or turn to self-induced or illegal back-alley abortions. Some may even consider suicide.

Proponents of Prop. 85 say that teens in abusive situations can just seek a court waiver if they can't talk to their parents. But courts are already backlogged. There is a lot of red tape, and courts are hard to navigate, even for adults. As the North County Times said last year, this judicial bypass option "is a cruel joke; imagine a girl who is pregnant, probably poor, and perhaps suffering under an abusive parent ---- what are the odds she will call a lawyer or otherwise enter the judicial system?"

Ironically Prop. 85's backers have admitted this fact, and are now arguing that Planned Parenthood will escort these teens through the courthouse, and yet it is these same people who regularly attempt to block access to our health centers, intimidate our patients, eliminate our funding, outlaw our services, and recall judges who protect our rights. If they succeed, who then will ensure the safety of these vulnerable teens?

Scared, desperate teens don't need courthouses and judges. They need counseling and medical care without delay.

A recent study released by the New England Journal of Medicine found that after a law like Prop. 85 took effect, some pregnant teens waited months ---- until their 18th birthday ---- to seek care and then faced riskier second trimester abortions.

That's why doctors, nurses and teachers ---- including the California Medical Association, the California Nurses Association, the California Teachers Association, Planned Parenthood and the American Academy of Pediatrics, California ---- all oppose Prop. 85.

Sixteen-year-old Rachel Ely's parents had no idea what was wrong with her when they found her in a coma on her bed one morning. She was taken by ambulance to a hospital in Riverside, where doctors determined she was suffering from an overwhelming septic bacterial infection ---- caused by an abortion she obtained without her parents' knowledge. Rachel survived but was left partially paralyzed and in a wheelchair.

Rachel's story illustrates just one of the dangers of secret abortions: young girls trying to hide their abortions from their parents will ignore or not recognize signs of post-abortion complications and delay seeking treatment until it is too late.

Did the doctor who operated on Rachel know her medical history? Not likely.

Was his license under discipline? Likely. California Medical Board records show that many doctors who end up at abortion clinics have disciplinary actions in their history for incompetence, negligence and even sexual abuse of their own patients.

That's the reality for minors seeking secret abortions: a pregnancy test, 15 minutes of filling out forms, and then an abortion performed by a doctor the girl has never seen before and will never see again.

Proposition 85 will put an end to this dangerous situation in California in which girls even younger than 12 can get an abortion without a parent knowing. She can't get a flu shot, have a tooth pulled, or go on a field trip without a parent's consent, but she can be taken to the clinic by anyone: her boyfriend, her boyfriend's mother, an adult male predator.

This is not a scare tactic, as the opposition says. Young girls are impregnated by older men at an alarming rate in California. Secret abortions allow these predators to cover up their crimes and continue the exploitative relationship ---- or move on to other victims. The younger the girl, the more likely she was impregnated by an adult male predator.

Parents spend years protecting the physical, psychological and emotional well-being of their daughters. They care about her future. A young girl ---- pregnant, scared, possibly abandoned or pressured by an older boyfriend or adult predator ---- needs the care and protection of a parent. Ten minutes with an employee of a clinic is not a substitute.

More than 30 states already have parental involvement laws like Prop. 85. Their experience shows that these laws substantially reduce teen abortion and pregnancy rates, as well as rates of sexually transmitted disease, without danger or harm to minors. Despite the opposition's dire predictions of minors turning to illegal abortions, there is no evidence that any girl has suffered harm because of a parental involvement law. There is plenty of evidence that girls have suffered when parents weren't informed about secret abortions that left their vulnerable daughters in the same exploitative relationship in which they got pregnant in the first place.

For those cases where a minor lives in an abusive home, Prop. 85 provides a quick and confidential procedure by which she can obtain a waiver from the juvenile court. This procedure also ensures that an abusive situation is brought to the attention of Child Protective Services so the problem can be addressed, not covered up with a secret abortion. Though the opposition claims the judicial bypass procedure is unworkable and unrealistic, the experience of other states shows that minors are able to use the system, and about 10 percent of them do so.

Fortunately, most parents aren't abusive, and most girls aren't scared of a violent or extreme reaction by their parents. By far, the most common reason given by girls for not telling a parent is that they didn't want to hurt or disappoint them. That's understandable. What's not understandable is why we would let a young girl's wish not to disappoint her parents override what common sense tells us is good medical practice and sound public policy.

Opponents claim that Prop. 85 is an attack on abortion rights, as if abortion was one big package, take it or leave it ---- parents left in the dark, incompetent doctors, sexual predators and all. Prop. 85 on its face specifically prohibits it from being used to restrict abortion in any other way. "Protect choice" is just something the opposition shouts over its shoulder as it's running away from the real issue: protecting our daughters.

Restore the right of California's parents to protect their minor daughters' well-being. Vote yes on Prop. 85.

Testicular Torsion

Background: Testicular torsion is a urologic emergency and must be differentiated from other complaints of testicular pain because a delay in diagnosis can lead to loss of the testicle. In adolescent males, testicular torsion is the most frequent cause of testicle loss.

Pathophysiology: The typical testicle is covered by the tunica vaginalis, which attaches to the posterolateral surface of the testicle and allows for little mobility.

In patients who have an inappropriately high attachment of the tunica vaginalis (ie, bell clapper deformity), the testicle can rotate freely on the spermatic cord within the tunica vaginalis (intravaginal testicular torsion). This congenital anomaly is present in approximately 12% of males, 40% of which have the abnormality in the contralateral testicle as well. The bell clapper deformity allows the testicle to twist spontaneously on the spermatic cord, causing venous occlusion and engorgement, with subsequent arterial ischemia causing infarction of the testicle. Experimental evidence indicates that 720° torsion is required to compromise flow through the testicular artery and result in ischemia.

In the neonatal age group, the testicle frequently has not yet descended into the scrotum and becomes attached within the tunica vaginalis. Additionally, this mobility of the testicle predisposes it to torsion (extravaginal testicular torsion).

Frequency:

In the US: Incidence of torsion in males younger than 25 years is approximately 1 in 4000. Torsion more often involves the left testicle.
Mortality/Morbidity: This urologic emergency requires prompt diagnosis, immediate urologic referral, and rapid definitive treatment for salvage of the testicle.

A salvage rate of 80-100% is found in patients who present within 6 hours of pain.
After 6-8 hours, the salvage rate markedly decreases, and it is near 0% at 12 hours.
Sex: Testicular torsion affects males only.

Age: Testicular torsion most often is observed in males younger than 30 years, with most aged 12-18 years. The peak age is 14 years, although a smaller peak also occurs during the first year of life.

Intussusception

Background: Intussusception is the telescoping or prolapse of one portion of the bowel into an immediately adjacent segment. Contrast enema can reduce the intussusception in approximately 75% of cases.

Pathophysiology: Intussusception most commonly occurs at the terminal ileum (ie, ileocolic). The telescoping proximal portion of bowel (ie, intussusceptum) invaginates into the adjacent distal bowel (ie, intussuscipiens).

The mesentery of the intussusceptum is compressed, and the ensuing swelling of the bowel wall quickly leads to obstruction. Venous engorgement and ischemia of the intestinal mucosa cause bleeding and an outpouring of mucous, which results in the classic description of red "currant jelly" stool.

Most cases (90%) are idiopathic, with no identifiable lesion acting as the lead point or pathological apex of the intussusceptum.

Frequency:

In the US: Intussusception is the predominate cause of intestinal obstruction in persons aged 3 months to 6 years. The estimated incidence is 1-4 per 1000 live births.
Mortality/Morbidity: Most patients recover if treated within 24 hours.

Mortality with treatment is 1-3%. If left untreated, this condition is uniformly fatal in 2-5 days.
Recurrence is observed in 3-11% of cases. Most recurrences involve intussusceptions that were reduced with contrast enema.
Sex:

Overall, the male-to-female ratio is approximately 3:1.
With advancing age, gender difference becomes marked; in patients older than 4 years, the male-to-female ratio is 8:1.
Age: Intussusception is most common in infants aged 3-12 months, with an average age of 7-8 months.

Two thirds of the cases occur before the patient's first birthday.
Intussusception occurrence is rare in persons younger than 3 months, and it becomes less common in persons older than 36 months.

History:

The typical presentation is a previously healthy infant boy aged 6-12 months with sudden onset of colicky abdominal pain with vomiting.
Paroxysms of pain occur 10-20 minutes apart.
Initially, loose or watery stools are present concurrent with vomiting and, within 12-24 hours, blood or mucous is passed rectally.
Early in the course, the patient appears completely well between the episodes of abdominal pain.
Lethargy may dominate the initial presentation. However, lethargy usually occurs later in the process.
The classic triad of colicky abdominal pain, vomiting, and red currant jelly stools occurs in only 21% of cases.
Physical:

Usually, the abdomen is soft and nontender early, but it eventually becomes distended and tender.
A vertically oriented mass may be palpable in the right upper quadrant.
Currant jelly stools are observed in only 50% of cases.
Most patients (75%) without obviously bloody stools have stools that test positive for occult blood.
Fever is a late finding and is suggestive of enteric sepsis.
Causes: Most cases are idiopathic. In neonates and in patients older than 3 years, a mechanical lead point usually can be found.

DESCRIPTION

Alinia Tablets and Alinia for Oral Suspension contain the active ingredient, nitazoxanide, a synthetic antiprotozoal
agent for oral administration. Nitazoxanide is a light yellow crystalline powder. It is poorly soluble in ethanol
and practically insoluble in water. Chemically, nitazoxanide is 2-acetyloxy-N-(5-nitro-2-thiazolyl)benzamide.
The molecular formula is C12H9N3O5S and the molecular weight is 307.3. The structural formula is:

Alinia Tablets contain 500 mg of nitazoxanide and the following inactive ingredients: maize starch, pregelatinized corn starch, hydroxypropyl methylcellulose, sucrose, sodium starch glycollate, talc, magnesium
stearate, soy lecithin, polyvinyl alcohol, xanthan gum, titanium dioxide, FD&C Yellow No. 10 Aluminum Lake,
FD&C Yellow No. 6 Aluminum Lake, and FD&C Blue No. 2 Aluminum Lake.

Alinia for Oral Suspension, after reconstitution, contains 100 mg nitazoxanide per 5 mL and the following
inactive ingredients: sodium benzoate, sucrose, xanthan gum, microcrystalline cellulose and carboxymethylcellulose sodium, anhydrous citric acid, sodium citrate dihydrate, acacia gum, sugar syrup, FD&C Red #40
and natural strawberry flavoring.

CLINICAL PHARMACOLOGY
Absorption: Following oral administration of Alinia Tablets or Oral Suspension, maximum plasma concentrations
of the active metabolites tizoxanide and tizoxanide glucuronide are observed within 1-4 hours. The parent
nitazoxanide is not detected in plasma. Pharmacokinetic parameters of tizoxanide and tizoxanide glucuronide are
shown in Tables 1 and 2 below.

Alinia for Oral Suspension is not bioequivalent to Alinia Tablets. The relative bioavailability of the suspension
compared to the tablet was 70%.

Effect of Food: When Alinia Tablets are administered with food, the AUCt of tizoxanide and tizoxanide glucuronide
in plasma is increased almost two-fold and the Cmax is increased by almost 50%.

When Alinia for Oral Suspension was administered with food, the AUCt of tizoxanide and tizoxanide glucuronide
increased by about 45%-50% and the Cmax increased by ·10%.

Alinia Tablets and for Oral Suspension were administered with food in clinical trials and hence they are
recommended to be administered with food (see DOSAGE AND ADMINISTRATION).

Multiple Dosing: Following oral administration of a single Alinia Tablet every 12 hours for 7 consecutive days,
there was no significant accumulation of nitazoxanide metabolites tizoxanide or tizoxanide glucuronide
detected in plasma.

Distribution: In plasma, more than 99% of tizoxanide is bound to proteins.

Metabolism: Following oral administration in humans, nitazoxanide is rapidly hydrolyzed to an active metabolite,
tizoxanide (desacetyl-nitazoxanide). Tizoxanide then undergoes conjugation, primarily by glucuronidation.
In vitro metabolism studies have demonstrated that tizoxanide has no significant inhibitory effect on cytochrome
P450 enzymes.

Elimination: Tizoxanide is excreted in the urine, bile and feces, and tizoxanide glucuronide is excreted in urine and
bile. Approximately two-thirds of the oral dose of nitazoxanide is excreted in the feces and one-third in the urine.

Special Populations

Patients with Impaired Hepatic and/or Renal Function: The pharmacokinetics of nitazoxanide in patients with
impaired hepatic and/or renal function has not been studied.

Geriatric Patients: The pharmacokinetics of nitazoxanide in geriatric patients has not been studied.

Pediatric Patients: The pharmacokinetics of nitazoxanide following administration of Alinia Tablets in pediatric
patients less than 12 years of age has not been studied. The pharmacokinetics of nitazoxanide following
administration of Alinia for Oral Suspension in pediatric patients less than 1 year of age has not been studied.

MICROBIOLOGY
Mechanism of Action

The antiprotozoal activity of nitazoxanide is believed to be due to interference with the pyruvate:ferredoxin
oxidoreductase (PFOR) enzyme-dependent electron transfer reaction which is essential to anaerobic energy
metabolism. Studies have shown that the PFOR enzyme from Giardia lamblia directly reduces nitazoxanide by
transfer of electrons in the absence of ferredoxin. The DNA-derived PFOR protein sequence of Cryptosporidium
parvum appears to be similar to that of Giardia lamblia. Interference with the PFOR enzyme-dependent electron
transfer reaction may not be the only pathway by which nitazoxanide exhibits antiprotozoal activity.

Activity in vitro

Nitazoxanide and its metabolite, tizoxanide, are active in vitro in inhibiting the growth of (i) sporozoites and
oocysts of Cryptosporidium parvum and (ii) trophozoites of Giardia lamblia.

Drug Resistance

A potential for development of resistance by Cryptosporidium parvum or Giardia lamblia to nitazoxanide has
not been examined.

Susceptibility Tests

For protozoa such as Cryptosporidium parvum and Giardia lamblia, standardized tests for use in clinical
microbiology laboratories are not available.

INDICATIONS AND USAGE
Diarrhea caused by Giardia lamblia or Cryptosporidium parvum:

Alinia for Oral Suspension (patients 1 year of age and older) and Alinia Tablets (patients 12 years and older)
are indicated for the treatment of diarrhea caused by Giardia lamblia or Cryptosporidium parvum.

Alinia for Oral Suspension and Alinia Tablets have not been shown to be superior to placebo for the treatment of
diarrhea caused by Cryptosporidium parvum in HIV-infected or immunodeficient patients (see CLINICAL STUDIES).

CONTRAINDICATIONS

Alinia Tablets and Alinia for Oral Suspension are contraindicated in patients with a prior hypersensitivity to
nitazoxanide or any other ingredient in the formulations.

PRECAUTIONS
General: The pharmacokinetics of nitazoxanide in patients with compromised renal or hepatic function have not
been studied. Therefore, nitazoxanide must be administered with caution to patients with hepatic and biliary disease,
to patients with renal disease and to patients with combined renal and hepatic disease.

Information for Patients

Alinia Tablets and Alinia for Oral Suspension should be taken with food.

Diabetic patients and caregivers should be aware that the oral suspension contains 1.48 grams of sucrose per 5 mL.

Drug Interactions

Tizoxanide is highly bound to plasma protein (>99.9%). Therefore, caution should be used when administering
nitazoxanide concurrently with other highly plasma protein-bound drugs with narrow therapeutic indices, as
competition for binding sites may occur (e.g., warfarin). In vitro metabolism studies have demonstrated that
tizoxanide has no significant inhibitory effect on cytochrome P450 enzymes. Although no drug-drug interaction
studies have been conducted in vivo, it is expected that no significant interaction would occur when nitazoxanide
is co-administered with drugs that either are metabolized by or inhibit cytochrome P450 enzymes.

Carcinogenesis, Mutagenesis, Impairment of Fertility

Long-term carcinogenicity studies have not been conducted.

Nitazoxanide was not genotoxic in the Chinese hamster ovary (CHO) cell chromosomal aberration assay or
the mouse micronucleus assay. Nitazoxanide was genotoxic in 1 tester strain (TA 100) in the Ames bacterial
mutation assay.

Nitazoxanide did not adversely affect male or female fertility in the rat at 2,400 mg/kg/day (approximately 20
times the clinical adult dose adjusted for body surface area).

Pregnancy: Teratogenic Effects
Pregnancy Category B: Reproduction studies have been performed at doses up to 3,200 mg/kg/day in rats
(approximately 26 times the clinical adult dose adjusted for body surface area) and 100 mg/kg/day in rabbits
(approximately 2 times the clinical adult dose adjusted for surface area) and have revealed no evidence of
impaired fertility or harm to the fetus due to nitazoxanide. There are, however, no adequate and well-controlled
studies in pregnant women.

Nursing Mothers

It is not known whether nitazoxanide is excreted in human milk. Because many drugs are excreted in human
milk, caution should be exercised when nitazoxanide is administered to a nursing woman.

Pediatric Use

A single Alinia Tablet contains a greater amount of nitazoxanide than is recommended for pediatric dosing
and should therefore not be used in pediatric patients 11 years or younger. Alinia for Oral Suspension should
be used for dosing nitazoxanide in pediatric patients (see DOSAGE AND ADMINISTRATION).

Safety and effectiveness of Alinia for Oral Suspension in pediatric patients less than 1 year of age have not
been studied.

Geriatric Use

Clinical studies of Alinia Tablets and Alinia for Oral Suspension did not include sufficient numbers of subjects
aged 65 and over to determine whether they respond differently from younger subjects. In general, the
greater frequency of decreased hepatic, renal, or cardiac function, and of concomitant disease or other drug
therapy in elderly patients should be considered when prescribing Alinia Tablets and Alinia for Oral
Suspension. As stated in the PRECAUTIONS section, this therapy must be administered with caution to
patients with renal and or hepatic impairment.

HIV-Infected or Immunodeficient Patients

Alinia Tablets and Alinia for Oral Suspension have not been studied for the treatment of diarrhea caused by
Giardia lamblia in HIV-infected or immunodeficient patients. Alinia Tablets and Alinia for Oral Suspension
have not been shown to be superior to placebo for the treatment of diarrhea caused by Cryptosporidium
parvum in HIV-infected or immunodeficient patients (see CLINICAL STUDIES).

ADVERSE REACTIONS
Alinia Tablets: In controlled and uncontrolled clinical studies of 1,657 HIV-uninfected patients age 12 years
and older who received various dosage regimens of Alinia Tablets, the most common adverse events reported
regardless of causality assessment were: abdominal pain (6.6%), diarrhea (4.2%), headache (3.1%) and
nausea (3.0%). In placebo-controlled clinical trials using the recommended dose, the rates of occurrence of
these events did not differ significantly from those of the placebo. In placebo-controlled trials of HIV-uninfected
patients age 12 years and older who received Alinia Tablets for the treatment of diarrhea caused by Giardia
lamblia or Cryptosporidium parvum, less than 1% of patients discontinued therapy because of an adverse event.

Adverse events occurring in less than 1% of the patients age 12 years and older participating in clinical trials
of Alinia Tablets are listed below:
Body as a Whole: asthenia, fever, pain, allergic reaction, pelvic pain, back pain, chills, chills and fever, flu syndrome.
Nervous System: dizziness, somnolence, insomnia, tremor, hypesthesia.
Digestive System: vomiting, dyspepsia, anorexia, flatulence, constipation, dry mouth, thirst.
Urogenital System: discolored urine, dysuria, amenorrhea, metrorrhagia, kidney pain, edema labia.
Metabolic & Nutrition: increased SGPT.
Hemic & Lymphatic Systems: anemia, leukocytosis.
Skin: rash, pruritus.
Special Senses: eye discoloration, ear ache.
Respiratory System: epistaxis, lung disease, pharyngitis.
Cardiovascular System: tachycardia, syncope, hypertension.
Muscular System: myalgia, leg cramps, spontaneous bone fracture.

Alinia for Oral Suspension: In controlled and uncontrolled clinical studies of 613 HIV-uninfected pediatric
patients who received Alinia for Oral Suspension, the most frequent adverse events reported regardless of
causality assessment were: abdominal pain (7.8%), diarrhea (2.1%), vomiting (1.1%) and headache (1.1%).
These were typically mild and transient in nature. In placebo-controlled clinical trials, the rates of occurrence
of these events did not differ significantly from those of the placebo. None of the 613 pediatric patients discontinued therapy because of adverse events.

Oral Suspension are listed below:
Digestive System: nausea, anorexia, flatulence, appetite increase, enlarged salivary glands.
Body as a Whole: fever, infection, malaise.
Metabolic & Nutrition: increased creatinine, increased SGPT.
Skin: pruritus, sweat.
Special Senses: eye discoloration (pale yellow).
Respiratory System: rhinitis.
Nervous System: dizziness.
Urogenital System: discolored urine.

The adverse events seen in adult patients treated with Alinia for Oral Suspension were similar to those
observed in adult patients treated with Alinia Tablets.

OVERDOSAGE

Information on nitazoxanide overdosage is not available. In acute studies in rodents and dogs, the oral LD50
was higher than 10,000 mg/kg. Single oral doses of up to 4,000 mg nitazoxanide have been administered to
healthy adult volunteers without significant adverse effects. In the event of overdose, gastric lavage may be
appropriate soon after oral administration. Patients should be carefully observed and given symptomatic and
supportive treatment.

DIRECTIONS FOR MIXING ALINIA FOR ORAL SUSPENSION

Prepare a suspension at time of dispensing as follows: The amount of water required for preparation of the
suspension is 48 mL. Tap bottle until all powder flows freely. Add approximately one-half of the total amount
of water required for reconstitution and shake vigorously to suspend powder. Add remainder of water and
again shake vigorously.

The container should be kept tightly closed, and the suspension should be shaken well before each administration.
The suspension may be stored for 7 days, after which any unused portion must be discarded.

HOW SUPPLIED
Alinia Tablets are round, yellow, film-coated tablets debossed with ALINIA on one side and 500 on the other
side. Each tablet contains 500 mg of nitazoxanide. The tablets are packaged in HDPE bottles of 60 tablets and
blister cards of 6 tablets.

CLINICAL STUDIES
Diarrhea caused by Giardia lamblia in adults and adolescents 12 years of age or older:

In a double-blind, controlled study (Study 1) conducted in Peru and Egypt in adults and adolescents with
diarrhea caused by Giardia lamblia, a three-day course of treatment with Alinia Tablets administered 500 mg
BID was compared with a placebo tablet for 3 days. A third group of patients received open-label Alinia for
Oral Suspension administered 500mg/25mL of suspension BID for 3 days. A second double-blind, controlled
study (Study 2) conducted in Egypt in adults and adolescents with diarrhea caused by Giardia lamblia compared
Alinia Tablets administered 500 mg BID for 3 days to a placebo tablet. For both of these studies, clinical
response was evaluated 4 to 7 days following the end of treatment. A clinical response of ‘well’ was defined
as ‘no symptoms, no watery stools and no more than 2 soft stools with no hematochezia within the past 24
hours’ or ‘no symptoms and no unformed stools within the past 48 hours.’ The following clinical response
rates were obtained:

lamblia, a three-day course of treatment with nitazoxanide (100 mg BID in pediatric patients ages 24-47
months, 200 mg BID in pediatric patients ages 4 through 11 years) was compared to a five-day course of
treatment with metronidazole (125 mg BID in pediatric patients ages 2 through 5 years, 250 mg BID in pediatric
patients ages 6 through 11 years). Clinical response was evaluated 7 to 10 days following initiation of treatment
with a ‘well’ response defined as ‘no symptoms, no watery stools and no more than 2 soft stools with no
hematochezia within the past 24 hours’ or ‘no symptoms and no unformed stools within the past 48 hours.’

In a second double-blind, placebo-controlled study of nitazoxanide tablets conducted in Egypt in adults and
adolescents with diarrhea caused by Cryptosporidium parvum as the sole pathogen, clinical and parasitological
response rates showed a similar trend to the first study. Clinical response rates, evaluated 2 to 6 days following
the end of treatment, were 71% (15/21) in the nitazoxanide group and 42.9% (9/21) in the placebo group.

Some patients with ‘well’ clinical responses had Cryptosporidium parvum oocysts in their stool samples 4 to
7 days following the end of treatment. The relevance of stool examination results in these patients is
unknown. Patients should be managed based upon clinical response to treatment.

Diarrhea caused by Cryptosporidium parvum in pediatric patients 1 through 11 years of age:

In two double-blind, controlled studies in pediatric patients with diarrhea caused by Cryptosporidium
parvum, a three-day course of treatment with nitazoxanide (100 mg BID in pediatric patients ages 12-47

Some patients with ‘well’ clinical responses had Cryptosporidium oocysts in their stool samples 3 to 7 days
following the end of treatment. The relevance of stool examination results in these patients is unknown.
Patients should be managed based upon clinical response to treatment.

Diarrhea caused by Cryptosporidium parvum in AIDS patients:

A double-blind, placebo-controlled study did not produce clinical cure rates that were significantly different
from the placebo control when conducted in hospitalized, severely malnourished pediatric patients with
acquired immune deficiency syndrome (AIDS) in Zambia. In this study, the pediatric patients received a three
day course of nitazoxanide suspension (100 mg BID in pediatric patients ages 12-47 months, 200 mg BID in
pediatric patients ages 4 through 11 years) and were evaluated for response four days after the end of treatment.

Rotavirus Vaccine Manufacturing Agreement Between BIOVIRx and IDT. BIOVIRx and IDT announced today an agreement intended to develop the oral rotavirus vaccine RotaShield(R) for commercialization. Worldwide, rotavirus gastroenteritis causes more than 2 million hospitalizations and up to 600,000 deaths annually in infants and young children. BIOVIRx has an exclusive worldwide license for RotaShield(R) and is planning global commercialization upon appropriate regulatory approvals. IDT currently has existing vaccine production capacity for RotaShield(R) and has initiated construction of a new manufacturing facility for expanded production of the vaccine. The new facility will be located at the IDT complex in Dessau-Tornau, Germany.

Eating disorder frequently unreported

When many people hear the words anorexia, bulimia and binge-eating they recall headlines associated with rail-thin female celebrities battling these eating disorders. Few are aware of a fourth -- uncommon -- eating disorder known as "pica" (pike-eh), the consumption of nonfood substances.

The term pica is derived from the Latin word meaning "magpie" and reflects that bird's peculiar eating habits.

Little is known about this type of eating disorder because it is frequently unrecognized and underreported. Typically, it is observed in children, individuals with developmental disabilities and pregnant women. It rarely occurs in men for reasons no one can explain.

Specific types of pica reported:

Amylophagia: the eating of laundry starch or cornstarch. Some theorize the sweet taste and crunchy texture of flaked laundry starch attracts a small number of women to eat these products, especially during pregnancy. Laundry starch is made from unrefined cornstarch and some will substitute cornstarch if laundry starch is unavailable.

Geophagia: the consumption of earth, particularly that with a high percentage of clay. Scientists believe it is harmful, while others think there may be an adaptive benefit, since humans and animals have done it for thousands of years. One of the main concerns about this practice is the consumption of parasite eggs in the earth that could lay dormant for years in soil but when ingested become active in the digestive tract.

Other dangers associated with geophagia are damage to tooth enamel, lead poisonings and intestinal obstruction.

Pagophagia: another type of pica that consists of a person crunching on ice. According to "Nutrition Now" by Judith Brown, "This person may have a nine in 10 chance that they are iron deficient. Regular ice eating, to the extent of one or more trays of ice cubes a day, is closely associated with an iron-deficient state. Ice eating usually stops completely when the iron deficiency is treated."

Plumbism: the medical term for long-term eating of paint chips which contain lead. Many older homes, apartment buildings and other buildings contain lead-based paint and dried-up paint chips. Although this eating disorder has decreased greatly over the years since enactment of legislation that prohibits use of lead-based paint in homes, there still is a need to be aware of this disorder.

While the mystery of pica is unexplained, many hypotheses have been proposed from cultural, socioeconomic, organic and psychodynamic perspectives. Whatever the reasons for this eating disorder, early detection and education are needed to emphasize healthy nutritional practices instead.

Life on your own brings challenges, successes

I am an adult. It is somewhat hard to believe this as I am sitting here wearing my raspberry lip gloss and my hair in double braids. I'm grooving to pop star Teddy Gieger and the teen magazine I've been flipping through is open to the story about boys.

I am an adult, though. These past few months have made me grow up faster than ever. Perhaps it's because Mom is almost 1,500 miles away and can't take care of me the way she used to. She always was there to lean on when it came to responsibility, and I always was scared of having to take over my life completely after moving away from home.

But I have managed to do it all on my own since January and I must admit it's been quite the experience.

Setting up the electricity and Internet service in my name was a big step. I even had to fight with the phone company because they charged me more than they had initially promised. The fight didn't end up in my favor. They won, but offered me 50 percent off my first month's bill. It's a good lesson -- you probably can't win against big corporations, but if you're persistent enough, you might get something out of it.

Paying rent every month is another thing I have had to get used to. This is my very first real apartment. (I lived in on-campus apartments in college, so this is a bit different.) Rent comes out of my pocket now and I have to remember to pay it at the first of the month instead of with tuition like in my previous experience. It's working out and there's nothing better than walking into your apartment knowing you worked for it.

During my full-time job hunt I have managed to get full-time freelance work with multiple publications. With this, I've learned the art of negotiation. Many offers for freelance work have come my way (exciting, right?!), so some of them have had to be turned down because of lack of time. I used this opportunity to ask for a pay increase for my hourly rate, and with a confident pitch, my wish was granted! It's nice to be in the adult-world where hard work literally pays off.

With no full-time job, though, I had to find insurance on my own, which is kind of scary. But I found there are a lot of programs that companies offer for people right out of college (be sure to research all of them if this is your situation too). Choosing a primary-care physician is kind of exciting, though. This is when it really hits that you are in the grown-up world.

So, with all this new-found responsibility, instead of calling Mom and asking her what to do, I'm actually calling her to tell her of my adulthood successes. Does this mean I'm actually looking forward to doing taxes and worrying about my 401k? Well, as long as I still feel young at heart, then bring it on.

Intussusception

Intussusception occurs when a segment of bowel telescopes (similar to closing of a telescope) into a segment just distal to it. It is the most common cause of intestinal (bowel) obstruction (blockage) between 3 months and 6 years of age. The ileocolic (small bowel into the colon) intussusception is the most common type although it may occur anywhere in the small bowel and colon.

Anatomy

The demarcation between the duodenum (1st part of small bowel) and the jejunum (2nd part of small bowel) is the ligament of Treitz (see Surgery of the Duodenum) (Figure 1)

The jejunum makes up about one third of the proximal small bowel

The ileum is the distal two thirds of the small bowel

The jejunum, ileum, and associated mesenteries (supporting and suspending structures) are attached to the back wall of the abdomen. They are completely covered with peritoneum (single layer of cells that line the surface of the abdomen and bowel)

The ileum joins the cecum (first part of the colon) in the right lower quadrant at the ileocecal valve near the appendix

The small bowel possesses an extensive lymphatic network that aids in the absorption of nutrients. It drains from the bowel wall into adjacent lymph vessels and lymph nodes and ultimately getting into larger lymphatics that finally empty into the left subclavian vein. The lymphatics of the small bowel play a major role in immune response (response to infection)
Pathology

The telescoping process is known as intussusception (Figure 2)

The leading proximal segment (intussusceptum) almost always telescopes into the distal segment (intussuscipens)

There may be a leading edge in the form of a polyp, inverted appendiceal stump, Meckel's diverticulum (See Surgery of the Jejunum and Ileum) or tumor

The leading edge gets caught up in the downstream peristalsis (wavelike action of the bowel wall that propels food) and is pulled into the distal bowel

In most cases the cause is unknown but viruses are thought to induce hyperplasia (increased size) of Peyer's patches (lymphoid tissue) in the end of the ileum
